29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Raw material mixing, coloring, and bagging/palletizing area(s): On or prior to 1/6/15, energy control (lockout/tagout) procedures were not documented for equipment, including but not limited to: Premier tech bagger, raw material hoppers, shaker unit, coloring machine, palletizer and all associated conveyors and equipment with multiple hazardous energy sources. (b) Raw material hopper 1, line 1: On or about 1/5/15, energy control (lockout/tagout) procedures were not utilized while an employee was performing maintenance on the head pulley of the conveyor with conveyor running and guard(s) removed. (c) Coloring Machine/Shaker: On or about 2/12/15, energy control (lockout/tagout) procedures were not used while employees were cleaning in/around the tail pulley of the coloring machine conveyor with guard removed and motor disconnect off but not locked out. |
