(b) Raw material hopper 1, line 1: On or about 1/5/15, energy control (lockout/tagout) procedures were not utilized while an employee was performing maintenance on the head pulley of the conveyor with conveyor running and guard(s) removed. (c) Coloring Machine/Shaker: On or about 2/12/15, energy control (lockout/tagout) procedures were not used while employees were cleaning in/around the tail pulley of the coloring machine conveyor with guard removed and motor disconnect off but not locked out. |
