The employee was using a harness that was not attached to an anchor point; on or about 10/18/16. This employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1), which was contained in OSHA inspection number 1001731 citation number 1, item number 2, issued on October 23, 2014 and was affirmed as a final order on November 17, 2014, with respect to a workplace located at 240 Brook Street, West Sayville, NY 11796.
