29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) On or about February 8, 2024, at the worksite 6106 State Highway 28, Fly Creek, NY 13337: Employees were utilizing a tubular welded frame scaffold which did not have a guardrail system on its ends. Employees exposed to falls of 11.5 ft. Simple Integrity was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.451(g)(1), which was contained in OSHA inspection number 1545957, citation number 1, item number 1 and was affirmed as a final order on 12/22/2021, with respect to a workplace located at 39 Chestnut Street, Cooperstown, NY 13326. |

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: a) On or about February 8, 2024, at the worksite 6106 State Highway 28, Fly Creek, NY 13337: Employees were engaged in roofing activities on a 12:12 pitch roof. The employer failed to protect employees from falling by use of guardrail systems, safety net systems, or personal fall arrest systems while working at the rake edge. Employees exposed to falls up to 15 feet from the rake edges. Simple Integrity was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1926.501(b)(13), which was contained in OSHA inspection number 1545957, citation number 1, item number 2 and was affirmed as a final order on 12/22/2021, with respect to a workplace located at 39 Chestnut Street, Cooperstown, NY 13326. |
