Cross braces were used as a means of access. Location: 959 41st Street, Brooklyn, NY. On or About: 07/16/2014 a) Employees worked on a tubular welded frame scaffold which was not provided with a safe means of access. Employees climbed the frames and cross braces to access work platforms. Employees exposed to a fall of approximately 40 ft. |
