While descending, the basket of the cherry picker nicked some thermal system insulation piper covering. Some insulation fell into the basket and onto the floor below. Employer failed to determine employee's exposure concentration. b) Y-9 Area - On or about 1/11/13, employer's supervisor required one or more employee(s) to remove the debris containing thermal system insulation.
