The employee was walking/working along the edge of the unprotected second floor, wearing a harness, but not tied off to an anchor point. b) Building A - On or about September 4, 2020 and at times prior thereto, an employee was exposed to fall hazards while framing the second floor of the building using an inadequate anchor point location which would not limit his fall to less than 6'. The employee was working from the second floor with his self-retracting lanyard anchored below the floors steel support beam. c) Building A - On or about September 4, 2020 and at times prior thereto, an employee was exposed to fall hazards while framing the second floor of the building using an inadequate anchor point location which would not limit his fall to less than 6'. Employees were accessing framing materials that were stored adjacent to a 72 inch wide framed door opening. No guardrails were installed on this opening. (b) Cutting Area, Northwest Side of the Building - on or about September 11, 2013 - a fall hazard of approximately 11 feet, 10 inches from the wall opening to the concrete pad existed on site. Employees were cutting framing lumber adjacent to a 72 inch wide framed door opening. No guardrails were installed on this opening. (c) East Side of the Building - on or about September 11, 2013 - a fall hazard of approximately 12 feet, 3 inches existed on site. Employees were cutting framing materials adjacent to a 72 inch wide framed window opening. No guardrails were installed on this opening. (d) Right Side Stairwell Opening - on or about September 11, 2013 - the front section of this opening on the second story floor was not equiped with a guardrail system. The stairs were not installed. Elevation to the first story floor was approximately 11 feet, 10 inches. |
