Citation text line | OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed by hazards. Site: 552 West 182nd Street New York NY On or about 7/23/16 a) An employee operated the dumbwaiter system. Doors to the hoistway were open when the dumbwaiter was in operation. Employees were exposed to the hazard of being struck by or crushed by the dumbwaiter.. Among other methods, several feasible and acceptable abatement methods to correct hazards are to: Comply with ASME A17.1-2016/CSA B44-13, Safety Code For Elevators and Escalators (Includes Requirements for Elevators, Escalators, Dumbwaiters, Moving Walks, Material Lifts, and Dumbwaiters With Automatic Transfer Devices) Section 7.2 Electric And Hand Dumbwaiters Without Automatic Transfer Devices states that Car enclosures and car doors and gates shall comply with 2.14,. Section 2.14.1.4.6 page 58 states, all hoistway doors shall be closed and locked and the car doors for each compartment closed before the car can be operated. Section 2.14.4.11 page 63 states, car doors or gates shall be considered to be in the closed position under the following condition: a) for horizontally sliding doors or gates, when the clear open space between the leading edge of the door or gate and the nearest face of the jamb does not exceed 50 mm (2in.) except where car doors are provided with a car door interlocks, 10 mm (0.375 in). Section 2.12.2.2 page 48 and section 2.12.3.2 also refers to Section 2.14.4.6 and to section 2.14.4.11 "car doors or gates shall be in the closed position). Section 7.2.1.2 page 251 states, Car door or gates shall be provided at entrances to the car, shall guard the full width of the opening and shall conform to 7.2.1.2.1 through 7.2.1.2.12. Section 7.2.1.2.2 page 251 states, Each door or gate shall be equipped with a contact that will prevent operation of the driving machine, unless the door gate panels is in the closed position as defined in 2.12.2.2 or 2.12.3.2. Section 7.2.1.2.12 page 251 states, Where car door or gate horizontal structural members are not fixed to the moveable panel, they shall not be capable of entering into the access door area. |
