An inspection shall be conducted by the competent person prior to the start of work and as needed throughout the shift. a) The employer did have competent person inspections of excavation activities. Employer excavated a 10 ft deep pit in the cellar without installing shoring, resulting in undermine of adjacent floor and wall footings; and without a guardrail around the pit for fall protection.
