Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees due to falls. On or about June 5, 2015 Location: 267 6th Street, Brooklyn, NY 11212 a). Employees engaged in widow cleaning operations at a 12 story residential structure were not trained in safe working procedures and proper use of equipment in accordance with ANSI/ASSE Z359.1-2007 Safety Requirement's for Personal Fall Arrest Systems. 1) Employees were using vertical life lines that were not secured to the building independent of each other as required by ANSI/ASSE Z359.1-2007, section 7.2.3 2) Employees were using vertical life line ropes that were placed over sharp roof edges without any protection to prevent abrasion or cutting of the ropes, ANSI/ASSE Z359.1-207 section 7.2.12.7. 3) Employees accessed the work areas by climbing down and moving with the personal fall arrest system and not a descent device system.
