29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Facility - Production Area: On, or about, 12/21/2015 employee conducts maintenance on machinery and other employees operate machines, without an energy control program in place to ensure that procedures, training and periodic inspections were implemented for the prevention of injuries caused by the released of stored energy, including electrical and pneumatic sources. Machines included, but may not be limited to: 1) Inserter Machine, 6 x 9 - Aisle # 8; 2) Inserter Machine, 6 x 9 - Aisle # 9; 3) Inserter Machine, 6 x 9 - Aisle # 10; 4) Inserter Machine, 6 x 9 - Aisle # 11.
