29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 06/20/16, on a construction site at a house undergoing renovations at 90 Crosby Blvd., Amherst,NY: Employees engaged in construction work including demolition, roofing & siding work , and general renovation work were not trained to recognize potential hazards on this type of jobsite, including but not limited to falls from roofs, ladders, skylights, stairs & scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. Crossbraces were used as a means of access: On or about 06/20/16, on a construction site at a house undergoing renovations at 90 Crosby Blvd., Amherst,NY. A ladder jack scaffold was erected with the brackets on the outside (street side) of the support ladders.
