The side rails only extended 1 foot above the upper landing surface, the ladder wasn't secured at the top, and a grasping device wasn't provided. |

(a) Front of Building - on or about March 16, 2012 - no top guardrails were installed for the window opening. Elevation to the ground was approximately 12 feet, one inch. (b) Center of Building - First Story Floor to the Basement - no guardrials or covers were installed at the basement opening. Elevation between these two floors was approximatley 10 feet, one inch. |
