Employees operated the forklift in high wind condition to lift a panel that is about 15 feet by 10 feet. Guardrails were removed from the edge of the building and employees were not tied off to fall arrest anchors during the installation process. On or about 11/21/18. Vescom Systems, LLC was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1200138, citation number 1, item number 2 and was affirmed as a final order on 04/18/17, with respect to a workplace located at Villa BXV Kensington & Sagamore Rd, Bronxville, NY 10708. |
