(a) Yates village building G, during roof tear-off and repair: On or about 5/11/16, extension cord used to power a porter-cable pancake compressor was not used with GFCI protection, exposing employees to a potential shock hazard. Cord was connected to a 120V receptacle inside the mechanical room of the building. (b) Yates village building G, during roof tear-off and repair: On or about 5/11/16, extension cord used to power a circular saw on the roof deck was not used with GFCI protection, exposing employees to a potential shock hazard. Cord was connected to a 120V receptacle inside a vacant apartment.
