ACT is based on the philosophy that each individual has a need and capacity for socialization and that being involved in community care helps prevent depression, loneliness, hospitalizations, and premature institutionalization.
ACT is a structured day program designed to provide meaningful, therapeutic, and age-appropriate activities for participants. ACT provides caregivers with respite and helps to alleviate some of the Care Giver’s daily responsibilities. The ACT program is therapeutic and helps to support families in their caregiving efforts.
Our goal is to provide a welcoming, home-like setting with warm, friendly, well-trained staff and volunteers who support the functioning of each participant. ACT’s goal is to improve or maintain skills and abilities, and to provide a safe environment that recognizes the dignity of each participant.
ACT program is developed for participants to be able to use community-based services to exercise their right to person-centered care. ACT understands that our member’s personal choice is important, and we encourage the right for each person to be able to achieve their individualized goals.
