There were multiple racks and they were constantly moving and left blocking exit access for employees towards the back of the rooms. Exit access was completely obstructed when the hot oven doors on the left hand side of the building were open with sheet pan racks in the walkway. |

On or about January 24, 2019, three horizontal mixers were unguarded while two employees added ingredients over the bowl while the agitator was in motion. One employee stood over the mixture watching bowl while other employee added the ingredients. Both employees stood directly over the unguarded mixers while the agitator was in motion. |
