29 CFR 1910.134(d)(1)(iii):The employer did not identify and evaluate the respiratory hazard(s) in the workplace; this evaluation shall include a reasonable estimate of employee exposures to respiratory hazard(s) and an identification of the contaminant's chemical state and physical form. Where the employer could not identify or reasonably estimate the employee exposure, the employer did not consider the atmosphere to be IDLH: a) Seat Line, demold area - On or about 11/16/21, the employer did not evaluate the respiratory hazard when employees were exposed to an unknown airborne concentration of toluene 2,4 diisocyanate (TDI) when instructed to clean the molds following an incomplete chemical reaction resulting in a "TDI/iso rich" condition due to a mis-pour caused by a system error. On 11/16/21, two employees became ill as a result of exposure to TDI while performing this task and were sent to the hospital. ABATEMENT CERTIFICATION REQUIRED |
