Employees were not provided with training in regards to the recognition and avoidance of hazards associated with scaffold use. Abatement certification is required for this item. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about 9/13/2017, Second floor deck: Employees were accessing the deck while walking on a wooden beam with no fall protection being utilized. Employees were exposed to falls of 15 feet to the exterior and ten feet to the interior. b) On or about 9/13/2017, Second floor deck: Employees were placing decking without utilizing any fall protection. Employees were exposed to falls of ten feet. c) On or about 9/13/2017, Second floor deck: Employees were installing exterior sheathing in an area which was lacking mid rails.
