Citation text line 29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired: On or about and times prior to: 8/27/19 Location: 1st to 2nd floor a) Defective job-made ladder was not taken out of service or tagged in such a way to prevent workers from accessing floors under construction exposing workers to a fall hazard. Among other methods, feasible and acceptable means to abate the hazardous condition include: 1. Institute a comprehensive inspection program to identify all job made wooden ladders. Inspect the ladders for defects including, but not limited to, missing filler blocks, crack, damage, rot, etc. 2. Complying with ANSI A14.4, American National Standard Safety Requirements for Job Made Wooden Ladders, sections 4.3 Ladder width, 4.5.1 Cleat attachment and 4.5.2 Filler Blocks or replacing job-made wooden ladders with metal, wooden, or fiberglass ladders manufactured in accordance with applicable ANSI standards. 3. The ladders width shall be at least 16 inches, but no more than 20 inches 4. Cleats shall be attached to the narrow face of each side rails. 5. Inserting filler blocks (of the same thickness as the rungs/cleats) between the rungs/cleats. Filler blocks shall be butted tightly against the underside of each rung/cleat.
