Location: a) On the pump-jack scaffold platform at the jobsite: On or about May 25, 2021, an employee worked on a pump-jack scaffold platform without the use of any personal fall arrest systems or guardrail system. The employee was exposed to falling approximately 15 feet from the platform to the lower level. |
