On the ladder jack scaffold, on the North side of 273 Front Street, employees used an extension ladder to access the scaffold platform approximately 18 feet from ground where that ladder did not extend at least 3 feet above the point of access. Those ladders were placed at the direction of the Site Foreman who did not recognize the incorrect use of three ladders. Abatement certification must be submitted for this item. |
