NOVEL POLYMER ADDITIVES FOR INFECTION CONTROL INTRAVENOUS CATHETERS - PROJECT SUMMARY CENTRAL LINE-ASSOCIATED BLOODSTREAM INFECTIONS (CLABSIS) COST THE HEALTHCARE SYSTEM AROUND $2 BILLION ANNUALLY WITH AN ESTIMATED >80,000 INCIDENTS EACH YEAR IN US INTENSIVE CARE UNITS (ICUS). THAT IS NOT SURPRISING WITH OVER 5 MILLION CENTRAL VENOUS CATHETERS (CVCS) USED ANNUALLY IN THE UNITED STATES AND AN ESTIMATED 27 MILLION UNITS SOLD GLOBALLY. MOST CVC-ASSOCIATED INFECTIONS ARE CAUSED BY MICROORGANISMS INTRODUCED FROM PATIENT SKIN DURING CATHETER INSERTION. THESE MICROORGANISMS CAN ALSO GET INTO THE LUMEN DURING MANIPULATION AND ATTACH TO DEVICE SURFACES, ENHANCE BIOFILM GROWTH AND COMPLICATIONS AND RESULT IN INFECTIONS. CURRENTLY, THERE ARE TWO MAIN APPROACHES TO REDUCING CVC-RELATED INFECTIONS EITHER (1) ADDING ANTIMICROBIAL FUNCTIONALITY OR (2) ADDING ANTIFOULING FUNCTIONALITY. OUR APPROACH COMBINES ANTIFOULING AND ANTIMICROBIAL KILLING FUNCTIONALITIES TO PREVENT CATHETER-ASSOCIATED INFECTIONS AND THROMBOSIS INTO A NEW POLYMER ADDITIVE PRODUCT (HALOADDTM) THAT CATHETER MANUFACTURERS CAN BLEND WITH THERMOPLASTIC POLYURETHANE (TPU). WE ARE AWARE OF NO OTHER SIMILAR APPROACH THAT COMBINES ANTIMICROBIAL AGENTS WITH ANTI-INFLAMMATORY AND/OR ANTI-ADHESION AGENTS TO ADDRESS THE COMPLEX PROBLEM OF CLABSI. THIS PHASE I EFFORT IS GEARED TOWARDS ANSWERING BASIC PERFORMANCE, SAFETY, AND EFFICACY ISSUES. WE PROPOSE TO OPTIMIZE POLYMER BLENDING PARAMETERS TO DEVELOP HALOADD-MODIFIED PU MATERIAL WITH MAXIMUM ANTIMICROBIAL AND ANTIFOULING EFFICACY WITHOUT IMPACTING CRITICAL MECHANICAL PROPERTIES, DETERMINE IN VITRO ANTI-THROMBIN AND ANTI- BIOFILM PROPERTIES, AND VERIFY THE BIOCOMPATIBILITY AND STABILITY OF HALOADD-BLENDED PU FOR THE CVC APPLICATION.

NON-COATING ANTI-MICROBIAL, ANTI-HOST PROTEIN DEPOSITION, ANTI-INFLAMMATORY URINARY CATHETER - ABSTRACT: CATHETER-ASSOCIATED URINARY TRACT INFECTION (CAUTI) IS THE MOST COMMON TYPE OF HEALTHCARE-ASSOCIATED INFECTION, HOWEVER CURRENTLY THERE IS NO EFFECTIVE TECHNOLOGY TO ADDRESS THE PROBLEM. ANTIMICROBIAL (ANTI-BIOFILM) FUNCTIONALITY ON THE CATHETER SURFACE IS AN IMPORTANT FIELD THAT HAS GAINED A LOT OF ATTENTION; HOWEVER, COMMERCIAL APPLICATIONS ARE LIMITED DUE TO A LACK OF INFECTION CONTROL OUTCOMES (SILVER-HYDROGEL COATING) OR IRRITATION/DISCOMFORT (NITROFURAL) IN CLINICAL TRIALS. RECENT RESEARCH HAS NOW SHOWN THAT BIOFILM FORMATION IS CAUSED BY PROTEIN DEPOSITION WHICH WILL ALSO CAUSE INFLAMMATION AND IMMUNOREACTION. THEREFORE, THE NEXT GENERATION TECHNOLOGY NEEDS BOTH ANTIMICROBIAL FUNCTIONALITIES ALONG WITH ANTI-IMMUNOREACTION CAPABILITIES TO PRODUCE A COMPOUND EFFECT FOR INFECTION CONTROL. OUR COMPANY INVENTED A NEW CATEGORY OF POLYMER ADDITIVE PRODUCT (HALOADDTM) THAT CAN BE COST-EFFECTIVELY BLENDED INTO EXISTING SILICON RUBBER RESINS TO PRODUCE SILICONE URINARY CATHETERS THAT CAN PROVIDE MULTIPLE FUNCTIONS SUCH AS ANTIMICROBIAL, ANTI-PROTEIN ADHESION AND ANTI-INFLAMMATION. THIS PHASE I EFFORT IS GEARED TOWARDS ANSWERING SOME BASIC FEASIBILITY, EFFICACY, AND SAFETY QUESTIONS ABOUT THIS APPROACH USING BOTH IN VITRO AND IN VIVO TESTING METHODS. WE PROPOSE THREE SPECIFIC AIMS: 1) OPTIMIZE POLYMER SYNTHESIS AND BLENDING PARAMETERS TO DEVELOP HALOADD-BLENDED SILICONE RUBBER FORMULAS WITH MAXIMUM ANTIMICROBIAL AND ANTI-FOULING EFFICACY; 2) DETERMINE UROPATHOGEN ATTACHMENT PREVENTION, BIOCOMPATIBILITY, AND STABILITY OF HALOADD-BLENDED SILICONE TUBES; 3) DEMONSTRATE IN VIVO THAT HALOADD CATHETER PROVIDES MEASURABLE ANTI-INFLAMMATION AND ANTI-BIOFILM OUTCOMES IN A MOUSE INFECTION MODEL. TECHNICAL SOLUTIONS ARE NEEDED THAT CAN ENHANCE CURRENT SYSTEMS AND REDUCE RISK OF FOOD-BORNE ILLNESS. BECAUSE MODULAR CONVEYOR BELTS REPRESENT THE SECOND MOST PROLIFIC CONTACT SURFACE WITHIN TYPICAL FOOD PROCESSING FACILITIES, THEY POSE A SIGNIFICANT RISK FOR CROSS-CONTAMINATION AND ARE A CHALLENGE TO EFFECTIVELY CLEAN. IN THIS PROPOSAL, HALOMINE, INC. PRESENTS HALOADD™, A NOVEL THERMOPLASTIC ADDITIVE FOR PREVENTING PATHOGEN CROSS-CONTAMINATION AND BIOFILMS ON MODULAR CONVEYOR BELT APPLICATIONS USING A POTENT RECHARGEABLE ANTIMICROBIAL PLUS ANTI-FOULING TECHNOLOGY.
