29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601 a) Employees engaged in masonry activities was accessing and egressing the third level scaffold platform that was bridged by (2) planks that crossed over approximately 4ft section to the wall. The employees were exposed to fall hazards of approximately 30ft to the ground below. Condition observed on or about 03/24/2022. |

On or about: March 24, 2022 Location: 131 Mamaroneck Ave., White Plains, NY 10601 a) (Unprotected planked access) employees engaged in masonry activities climbed onto leading edge wall to access (2) planked bridged that crossed over approximately 4ft section to third level scaffold platform. The employees were exposed to fall hazards of approximately 30ft to the ground below as a means of fall protection had not been utilized. Condition observed on or about 03/24/2022. |
