29 CFR 1926.34(c): Means of egress was not continually maintained free of all obstructions or impediments to full instant use in the case of fire or other emergency. a) Tower Crane barricade, center of jobsite at building foundation level: An egress door for the temporary wood barricade around the base of the tower crane was the only means of egress for a crane operator who worked inside the crane. The door was obstructed and could not be opened. Bracing installed for the metal formwork was erected and the operator could not open the door. Means of egress was not continually maintained free of all obstructions or impediments to full instant use in the case of fire or other emergency. On or about 05/21/13. Note: Because abatement of this violation is already documented in the inspection case file, the employer need not submit certification of abatement for this violation as normally required by 29 CFR 1903.19. |

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached. a) Ground floor, metal formwork deck for concrete pouring: An employee wore a safety harness and anchored his lanyard strap around a shoring beam of the metal formwork. The shoring beam was not designed to support to support and be used as an anchor for a personal fall arrest system. The system was not designed to support 5,000 pounds per employee attached to the beam. On or about 05/21/13. b) Ground floor, metal formwork deck for concrete pouring: Employees tied off to straps that were nailed to the metal formwork deck as fall arrest anchorages. The panels of formwork deck, which were used as anchorages for attachment of personal fall arrest equipment, were not designed to support 5,000 pounds per employee attached. The strpas were not installed per the manufacturers design and specs and anchorage requirements. On or about 05/21/13. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT CERTIFICATION OF ABATEMENT FOR THIS ITEM. FAILURE TO COMPLY MAY RESULT IN AN ADDITIONAL PENALTY OF $1,000 AS PER 29 CFR 1903.19. |
