Location: a) Supported scaffold on the north side of the building: On or about September 20, 2022, employees installed a winch hoist, for material hoisting, on the top tier platform of the scaffold. Subsequently, the employees removed two planks from the scaffold platform underneath the hoist, to provide an opening to hoist material. The employer did not have a competent person that inspect the scaffold before the work shift and after the hoist was installed and the planks were removed. |

There was no competent person to supervise and direct employees in scaffold alteration. |
