29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Scaffold frames were used as a means of access. Location: a) Supported scaffold on ground level of the jobsite: On or about April 23, 2024, employees were working on the fa�ade of the building while standing on the first-tier and second tier platforms of a four-tier-high supported scaffold. There was no safe means of access provided. The employees had to climb the scaffold frame and the crossbraces to get to the platform. The employees were exposed to falling up to approximately 12 feet to the lower level. |
