29 CFR 1910.269(a)(3)(i)(A): The host employer did not ensure that each of its employees were instructed in the hazardous conditions relevant to the employee's work that the contract employer was aware of as a result of information communicated to the contract employer by the host employer under paragraph (a)(3)(i) of this section: (a) Mount Ida Hydro Station, Control room level: On or about May 21, 2024, sub-contractor employees conducting maintenance on an Allis Chalmers MA-250C circuit breaker in a 4160-volt switchgear housing experienced an arc flash when the breaker contacted an energized cell during racking-in. The host employer did not inform the subcontractor of the characteristics of the host employers installation, including but limited to the nominal voltages of lines and equipment and maximum switching transient voltages. |
