Citation text line 29 CFR 1926.502(I)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees. Site: 148-18 Hillside Ave, Jamaica, NY 11435. On or about August 28, 2024, employees engaged in stripping plywood from dry concrete floors, and cleaning construction debris were exposed to falls due to two instances of floor opening covers that were not secured. a) 3rd floor opening measured approximately 4ft wide and 4ft long. b) 4th floor opening measured approximately 2ft wide and 4ft long. These hazards exposed employees to trip and fall hazards. Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.
