Citation text line 29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury. location: 620 Atlantic Avenue, Brooklyn, NY On or abut 2/24/2015 a) The employer did not have a written procedure in place to instruct its driver to recognize and avoid unsafe conditions associated with delivering, unloading and unstrapping material at construction sites nor did the employer adequately instruct its driver how to control hazards associated with that work. On 2/24/2015 a load of steel joist was unstrapped prior to communication between the driver and the lifting gang. The load was unstable and fell off the truck to the ground.
