Citation text line | 29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary: a) Finishing area, on or about 6/26/19: A finisher was overexposed to noise above 90 decibels (dBA) at a dose of 251% for an 8 hour time weighted average (8-hr. TWA) of 96.63 dBA during the 330 minute sampling period. A zero increment was included for the 150 minutes not sampled. The employee was not refitted and retrained on hearing protection after exhibiting a standard threshold shift (STS) on the 2019 audiogram. b) CCG area, on or about 6/26/19: Representative sampling showed that a CCG operator was overexposed to noise above 90 dBA at a dose of 135.6% for an 8-hr. TWA of 92.2 dBA during the 339 minute sampling period. A zero increment was included for the 141 minutes not sampled. The employee was not refitted and retrained on hearing protection after exhibiting a standard threshold shift (STS) on the 2019 audiogram. c) Finishing area, on or about 6/26/19: A finisher was exposed to noise above 85 dBA at a dose of 98.3% for an 8-hr. TWA of 89.8 dBA during the 363 minute sampling period. A zero increment was included for the 117 minutes not sampled. The employee was not refitted and retrained on hearing protection after exhibiting a standard threshold shift (STS) on the 2019 audiogram. d) Finishing area, on or about 6/26/19: Representative sampling showed that a finisher was exposed to noise above 85 dBA at a dose of 87.9% for an 8-hr. TWA of 89.1 dBA during the 362 minute sampling period. A zero increment was included for the 118 minutes not sampled. The employee was not refitted and retrained on hearing protection after exhibiting a standard threshold shift (STS) on the 2019 audiogram. |

Visible alarm could not be seen from the sides or the back of the containment opening. The audible alarm was 84 dBA at the door and below 67 dBA at the sides and the rear of the containment. b) Linacs 2, 450 Kv X-Ray system, on or about 10/17/19: Neither an audible nor visible alarm for the 450 Kv system was located so that employees working at the sides and the rear of the containment including, but not limited to, supervisors and non-entry employees were aware of an entry. Visible alarm could not be seen from the sides or the back of the containment opening. The audible alarm was imperceptible at the door and at the sides and the rear of the containment. Abatement certification must be submitted for this item. |
