OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazards by the load should the load slip off the hook, or the lifting device fails: a) Service Area, on or about 7/24/18: 2-Ton Contrx hoist, Model A41414 used to lift truck parts had a broken safety latch on the lifting hook. Among other methods one feasible and acceptable method to correct this hazard is to: a. Replace the broken safety clip or remove the hook from service per ASME B30.10-2014, Section 10-1-10.5; and b. Perform frequent and periodic inspections as required by ASME B30.10-2014, Section 10. b) Service Area, on or about 7/24/18: 2-Ton Contrx Hoist used to lift truck parts had not been inspected on a frequent and periodic schedule. Among other methods one feasible and acceptable method to correct this hazard is to: a. Perform frequent and periodic inspections as required by ASME B30.16-2017, Chapter 16-2. c) Service Area, on or about 8/24/18: Hydraulic transmission jacks had not been inspected on a frequent and periodic basis. The following deficiencies were noted: 1) The transmission jig had been modified and the employer had not performed a proof test prior to use; 2) The leveling handle was bent. Among other methods one feasible and acceptable method to correct these hazards is to: a. Have a qualified person or the manufacturer proof test the modified jack; and b. Replace the bent leveling handle; and c. Conduct frequent and periodic inspections of the jacks in accordance with ASME B30.1-2015, Chapter 1-2. |
