29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) 2nd Floor Willow Building Near Stairs - On or about June 20, 2012, a breaker panel box did not have a panel door, exposing live electrical components. b) 2nd Floor Willow Building at Time Clock - On or about June 20, 2012, an industrial control transformer did not have cover, exposing live electrical components. c) 2nd Floor Willow Building Next to Elevator in Staging Room - On or about June 20, 2012, a breaker panel box did not have a panel door or service panel cover, exposing live electrical components. d) 3rd Floor Willow Building Bubble Out Pouch Converting Area - On or about July 24, 2012, electrical wires were not contained for the Ro-An Polystar 4000, exposing live electrical components. |
